If you live in Staffordshire and Stoke-on-Trent and are having problems with you hearing, you may be able to self-refer into NHS hearing (audiology) services if you are eligible.

Normally, you would have to have an appointment with your GP in order to get a referral to these services. However, this option makes it easier, by giving you more ways to access the service and meaning that you may be able to bypass the need to visit your GP practice.

If you have urgent issues, you may still need to go to see your GP to get a referral. However, you can check whether you are eligible to self-refer by answering a few questions about your hearing problem.

To find out whether you are eligible to self-refer to local audiology services, you can complete this short form. Alternatively, you can download and print the form to complete it to find out if you can self-refer to local NHS hearing services. No one but you will see this form (unless you show it to anyone else), it is just a tool to check whether you are eligible to self-refer or not.

Once the form has been completed, if you are eligible, you can contact a provider of your choice by telephone or email to make an appointment. You may also be able to contact providers for help with completing the form. Please click here for provider contact details. If you are not eligible, please contact your GP practice for an appointment about your hearing.
