The My Health and Care App, sometimes known as the Personal Health Record, is a mobile app that you can download on your phone or tablet. It’s part of the One Health and Care program, and it lets you access important information about your health and social care whenever you need it, all in one place. 

With this app, you can: 

  • See a summary of your medical and social care records: You can check out important information about your health that your GP, and other professionals have written down. 
  • Keep track of your health and wellbeing: You can also use the app to record your own information, like how you’re feeling or any changes in your health. 
  • Get advice and support: If you need help or advice about your health, the app can connect you with helpful resources and support. 

If you’ve decided not to share your information through the One Health and Care program, you won’t be able to use the app. If you are happy to share your data and wish for your data to be viewable within One Health and Care you will need to contact your GP Practice and inform them of your decision.

    The My Health and Care App, sometimes known as the Personal Health Record is a mobile app that you can download on your phone or computer. It shows you a summary of your private health and care information whenever you have internet access.

    In the future, if you have a long-term illness, there might be special features just for you.

  • What is the difference between One Health and Care and the My Health and Care App?

    One Health and Care is the shared care record that holds information from different organisations where you get medical help. It's a special folder that only doctors and people who take care of you can look at. They use it to see all the important details about your health 

    The My Health and Care App or website that shows a summary of your health and care information. You can choose to download it on your phone or computer. You can also add some information to it yourself.

  • How do I get the My Health and Care App?

    The App is free to download from the App Store and Google Play. You can also access NHS App services from the browser on your desktop or laptop computer.

  • What is NHS login?

    NHS login is like a special key that lets you go into different websites and apps about health and care. You only need one email address and password to get in.

  • What information you need to set up an NHS login

    You will need: 

    • an email address 
    • a UK mobile phone number 

    Here's how you can set up an NHS login: 

    • NHS login services will send you an email with a special link. 
    • Click on the link in the email to confirm your email address. 
    • Go back to the website or app you want to use and enter your NHS login email address and password. 
    • They will ask for your mobile phone number. 
    • You'll receive a 6-digit security code in a text message on your phone. 
    • Enter this code to continue using the website or app. 
    • Every time you log in, you'll need to enter a security code, unless you choose to remember your device and stop receiving codes. 

    Right now, you must have a mobile phone to set up an NHS login. But don't worry! NHS Digital is working on finding a solution for people who don't have a mobile phone in the future.

  • Who can view my information in the My Health and Care app’?

    Only you can access your My Health and Care App. Your doctors and nurses have a special version called One Health and Care, but nobody else can see your information. If you put something in your record, your doctors and nurses won't be able to see it. If you need urgent help, contact the right emergency service like you normally would.

  • How do I log out of the My Health and Care App?

    To log out of the My Health and Care App, just find the little icon on the top right (if you're using an iPhone or iPad) or the top left (if you're using an Android phone or tablet) of the screen.  

    Once you see it, tap on it. A menu will appear, and from there, pick the option that says, 'Sign Out.' That's it! You'll be logged out and taken back to the page where you log in next time. 

  • How do I find my way around the My Health and Care App?

    My Health and Care is like a website or app that's easy to use. You start on the main page and can click on different sections to see stuff you want. If you want to go back to the main page, just press the 'Back' button or the 'Home' button. 

  • Why don’t I see all my medical record in the My Health and Care App?

    The My Health and Care App is like a summary of your important health information. It has different types of information, such as: 

    • Personal details and stuff about you, like your age and where you live. 
    • Appointments and appointments letters. You can see both future and past appointments, and even visits to the emergency room or hospital. 
    • Some partner organisations are working on making it possible for you to see your appointment letters online too. 
    • Medications: You can see the medicine your doctor prescribed for you, including if you need to take it regularly. You can also add any other medicine you take without a prescription in the "Over the Counter" section. 
    • Your health stuff, like if you have any allergies, your family's health history, any medical problems you have, and information about your lifestyle. It can even show things like your blood pressure. But remember, this information will only be there if your doctor has it in their records about you. 
  • Frequently Asked Questions
    • What if I can’t see my information? 

    First you should ask your GP practice if you have ever decided not to share your information. If you did, you can ask them to change that. If you can't see your GP data in the app, it might be because your GP doesn't share information with One Health and Care. But if you got treatment from other organisations that do share with One Health and Care, you'll still see your information from those places. 

    • What if any of my information is incorrect? 

    If you think there is a mistake or something missing in your information shown in the My Health section of the My Health and Care App, please get in touch with the relevant Health or Social Care organisation who provide the information. 

    •  What if I have changed my name or address? 

    It's important that you let all the organisations where you get healthcare and social care know about these changes.  That way, they can keep in touch with you without any problems.  Just call or reach out to each care provider and tell them your new information. 

    • Is my information protected? 

    Once you get the app, you'll need to make an NHS login and prove it's really you. If your device can read your fingerprint or recognise your face, you can use that instead of a password and code to log in every time. The app then connects safely to the My Health and Care App. Your information gets turned into a secret code, so it stays safe while it's being sent, and it's kept in a special UK data center approved by the UK government. None of your information in the My Health and Care App is saved on your computer, laptop, or phone. To keep things safe and private, make sure to keep your login details to yourself and don't tell anyone. If you want more details about your data, you can check the privacy notice in the settings of your My Health and Care App. 

    •  How do I connect my Garmin, Apple or Fitbit device? 

    You can easily access it from your home page. To link your Garmin or Fitbit wearable device to your phone or tablet, just tap on the icon at the top right (for Apple devices) or top left (for Android devices) of the menu on the home page. Then, tap on 'settings'. After that, tap on 'health services' and choose either GarminConnect or Fitbit, depending on what you have. Remember, if you connect your wearable device to the 'health services', you're choosing to upload your information from the device to the Personal Health Record. This means that the data might be used for other things as well. 

    • Can I delete information I have added to the My Health and Care App? 

    If you put in something wrong and saved it, don't worry! You can just delete it and fix it. You can either change what you put or just leave it empty and save it again. 

    • What do I do if I have a question or need help with the My Health and Care App? 

    You can find help by checking out the "Contact us" section in the app. It will tell you how to get support if you need it.  Another way is to use a feature in the app called "Remember to Ask." It lets you write down your questions and then you can ask your GP or other healthcare professional about them at your next appointment.
