  • What is my Personal Health Record?

    Personal Health Record is a website and an app you can download on to your personal device which will present to you a summarised view of your confidential health and care record wherever you have internet access.

    Below are examples of Personal Health Record will enable you do to subject to the information being made available by the appropriate organisations involved in your care: 

    • View a summary of your medical and social care records
    • Record information to self-manage your health and wellbeing
    • Access advice and support for your health and wellbeing

    In the future if you have been diagnosed with a long-term condition you may also have access to a condition specific modules. The Personal Health Record section provides more information regarding what data you would be able to view about yourself.

  • What is the difference between One Health and Care and the Personal Health Record?

    One Health and Care is the shared care record which displays information from organisations involved in your health and care, it is accessed by health and care professionals.

    • Personal Health Record is a summary view of your health and care record which can be viewed through a website or an app which you have the option to download to your chosen device. You have the ability to add certain types of information to the Personal Health Record service.

  • How do I get the app?

    The App is free to download from the App Store and Google Play.

    You can also access NHS App services from the browser on your desktop or laptop computer.

  • How do I access Personal Health Record?

    Once you have downloaded the App, access to Personal Health Record is via NHS Login. Click on the link within the app to login or create a new NHS login account.

  • What is NHS login?

    NHS login lets you access a variety of digital health and care websites/apps with just one email address and password.

  • What information you need to set up an NHS login

    You will need:

    • an email address
    • a UK mobile phone number

    What you need to do:

    NHS login services will send you an email containing a link. Select the link to confirm your email address. Then return to the website or app you want to access and enter your NHS login email address and password. Here, you will be asked to provide your mobile phone number.

    A 6-digit security code will be sent by text message to your mobile phone. You will be asked to enter this code to continue. A security code will be needed to log in every time, unless you select ‘remember this device and stop sending security codes’.

    Right now, you cannot set up NHS login without a mobile phone. NHS Digital are working on this and hope to offer a solution to users who do not own a mobile phone in the future.

  • Who can view my information in the Personal Health Record?

    Only you will have access to view your information in Personal Health Record. Your healthcare and social care professionals have access to a clinical version of the Personal Health Record – One Health and Care. No one else will have access to your information.

    Any information you enter in the Personal Health Record is saved to your record but will not be seen by a health or social care professional. Where you have an urgent or emergency health or social care requirement you should contact the appropriate urgent or emergency service using the normal method.

  • Why doesn’t the Personal Health Record show all my record?

    Personal Health Record is designed to give you a summary view of information from your record. The type of information available is:

    • Personal details and demographics
    • Appointments and appointment letters - future and past appointments together with A&E attendances and hospital admissions may also be viewed here. 
    • Some partner organisations are working to enable electronic appointment letters for you to view.
    • Medication - GP-prescribed medication, including repeat prescription and recent GP- provided vaccinations. There is also an opportunity to enter self-recorded medication within the ‘Over the Counter’ section. 
    • My Health - known allergies, family history, medical problems, lifestyle and measurements such as blood pressure will show here. Please note this will only be applicable if this information exists about you in your GP clinical system.
  • What if I can’t see my information?
    • You will need to check if you have ever chosen to not have your data shared by enquiring at your GP practice.  You are able to request a change in this status should you choose to.
    • If you are unable to see your GP data in the app, your practice may not be sharing information with One Health and Care, but you may have received care from partners sharing data with One Health and Care; if so you will only see your data from those organisations.


  • What if any of my information is incorrect?

    If you think there is a mistake or something missing in your information shown in the My Health section of the Personal Health Record, please contact the relevant Health or Social Care organisation who provide the information.

  • What if I have changed my name or address?

    It is important that you make all health and social care organisations that you receive care from aware of any changes to your personal details to ensure communication to you is not hindered.  Please contact each care provider to update your details.

  • Is my data safe?

    After you download the app, you will need to set up an NHS login and prove who you are. If your device supports fingerprint detection or facial recognition, you can use it to log in to the NHS App each time, instead of using a password and security code.

    The app then securely connects to Personal Health Record. Your information is encrypted so is completely secure when being transmitted and is stored in a UK data centre accredited by the UK government.

    None of your information in Personal Health Record is stored on your PC, laptop or mobile device.

    For your security and confidentiality please keep your login details secure and do not share with anyone.

  • How do I log out of the Personal Health Record?

    To log out of the Personal Health Record click the icon on the top right (for Apple devices) or top left (for Android devices) of the navigation bar on the home page. Then choose the ‘Sign Out’ option from the menu. You will then be logged out and return to the login page.

  • How do I navigate around the Personal Health Record?

    Personal Health Record uses familiar web and app behaviour for navigation. From the homepage click on a section you wish to view. To return to the homepage use the ‘Back’ button or ‘Home’ button.

  • How do I connect my Garmin, Apple or Fitbit device?

    It will be accessible from your home page.

    To connect your Garmin or Fitbit wearable device to your phone or tablet click the icon on the top right (for Apple devices) or top left (for Android devices) of the navigation bar on the home page and click on ‘settings’. Then click on ‘health services’ and select GarminConnect or Fitbit as appropriate.

    Please note if you connect your wearable device to the ‘health services’ you are proactively opting in to upload your information from your wearable device to the Personal Health Record so this data may also then be used for secondary use purposes.

  • Can I delete information I have added to Personal Health Record?

    You can delete a value if you have entered and saved it incorrectly; the value can be edited or the field left blank and then resaved.

  • What if I have a question or need support with my Personal Health Record?
    • Please see ‘Contact us’ within the app for details of how to get support.
    • Alternatively you could use a feature within the app named ‘Remember to Ask’ to make a note of your question to ask your health or social care professional at your next appointment.
