Midlands Partnership University NHS Foundation Trust (MPFT) is commissioned by Cannock Chase, East Staffordshire, South East Staffordshire and Seisdon Peninsula and Stafford and Surrounds Clinical Commissioning Groups (CCGs) to provide community assessment and support for children and young people up to the age of 19, and up to the age of 25 if the young person has an Education, Health and Care Plan (EHCP).

Learning Disability Register

Do you have a learning disability or support someone who does? By adding your name to the learning disability register at your GP practice you can get better healthcare – and you could have a free Annual Health Check. More information about the Learning Disability Register can be found on the Mencap website.

Annual health checks and health action plans

Everyone over 14 who is on the doctors Learning disability register has an annual health check.

This can help you stay well and helps by finding problems early so that can be sorted out for you. If you are worried about having this health check then let the practice know so that they can make changes for you, these are called reasonable adjustments. Guidance on Reasonable Adjustments

A ‘reasonable adjustment’ is when healthcare professionals make changes to how care is delivered for a person with a learning disability and complex needs, to make sure they get the healthcare they need. 

Reasonable adjustments are a legal requirement under the Equality Act, 2010. They are person-centred and may be different from person to person. Guides on how reasonable adjustments should be made to health services and adjustments to help people with learning disabilities to access services can be found on the Gov.uk website.
