A resource pack to further support care homes and care staff in recognising and responding to deterioration in their residents has been published by Staffordshire and Stoke-on-Trent Integrated Care System (ICS).

Developed by health and social care partners across the ICS, the pack is an updated version of the 2019 resource and brings together a suite of information, service details and support resources, as well as content on specific healthcare areas such as frailty, mental health conditions, and palliative and end of life care.

Heather Johnstone, Chief Nursing and Therapies Officer, Staffordshire and Stoke-on-Trent Integrated Care Board (ICB), said:

“Staffordshire and Stoke-on-Trent Integrated Care Board has worked with Health Innovation West Midlands (HIWM) and partners across our system to develop this pack, which is a useful tool for care homes and care staff – it brings together a lot of key information in one place for professionals to access.”

Dr Richard Harling, Staffordshire County Council’s Director for Health and Care, said: “It is important that care homes know how to get timely and appropriate support for their residents from the NHS. I hope that this new pack will help care homes spot changes in the health of their residents and get assistance from the right services.”

Peter Tomlin, Director of Adult Social Care and All Age Commissioning at Stoke-on-Trent City Council, said: “We are pleased to launch the Care Home Resource Pack across our Stoke-on-Trent homes, as it will be a vital tool in enabling the care sector to recognise deterioration at an early point and ensure proportionate and appropriate action is taken to safeguard the health and wellbeing of adults in care.

“The pack will also benefit any service that provides care and support to vulnerable adults and be a key component in reducing hospital admissions.”

The pack is available to access on the ICB website and is being distributed to care homes across Staffordshire and Stoke-on-Trent.
