One Health and Care (OHC) is a centralised electronic shared care record that contains patients’ health information. It allows multiple health providers across Staffordshire and Stoke-on-Trent to access and update the same information. OHC helps to ensure that that all healthcare providers have access to the most up-to-date and relevant information about patients which leads to a better coordination of care and patient outcomes.

OHC gives healthcare professionals a comprehensive view of the patient’s health history, encompassing information from social care to mental health. This depth of knowledge means a fuller, more accurate understanding of the patient’s condition and needs. It provides a richer, consolidated data source which enables healthcare professionals to make more informed, accurate, and safer care decisions. OHC offers quick, streamlined access to all pertinent patient data, significantly reducing the time spent on enquiries and increasing time devoted to actual patient care.

The benefits management team has identified a significant time saving of 450 hours per week across A&E departments and 600 minutes per month in social care which has translated to an increase in efficiency and improved patient care across health and social care.
