In partnership with University Hospitals of North Midlands (UHNM) we have developed a Children’s Health Passport, which is a document designed to support children and young people when they visit a hospital, clinic or go to respite care.

What is the Children’s Health Passport?

The passport provides healthcare professionals with important information about a child or young person in one place. It helps staff quickly understand:

  • Medical needs and conditions
  • Communication preferences
  • Sensory needs or additional support requirements
  • What helps them feel safe and comfortable
  • Important contacts and care plans

Why should the Children’s Health Passport be used?

Every child is unique. The Children’s Health Passport ensures that healthcare teams can:

  • Deliver more personalised care
  • Reduce the need to repeat information
  • Work in partnership with parents and carers
  • Make hospital visits less stressful
  • Improve safety and understanding

By sharing key information clearly and consistently, the passport helps us provide the best possible care experience for children and young people across Staffordshire and Stoke-on-Trent.
