Prepayment prescriptions for care leavers

Download the application form for NHS Staffordshire and Stoke on Trent ICB pre-paid prescription certificates for care leavers currently aged 18 – 25 (who would otherwise not be entitled to free prescriptions).

This offer is open to Staffordshire and Stoke on Trent Care leavers who are would otherwise not be entitled to free prescriptions.
